Tolérance et efficacité de l'association gemcitabine et nab-paclitaxel chez les sujets âgés atteints de cancer pancréatique non résécable : étude rétrospective et multicentrique

Abstract : Background: gemcitabine plus nab-paclitaxel is a standard chemotherapy for advanced pancreatic cancer but its safety and efficacy are ill-defined in patients ≥ 75 years. Methods: we included consecutive patients ≥ 75 years old (elderly group) with advanced pancreatic cancer treated with gemcitabine plus nab-paclitaxel in four French hospitals from January 2014 to May 2018. We constituted a control group of patients aged 65-75 (younger group). We compared the toxicity, overall survival (OS), progression-free survival (PFS) and objective response rate (ORR) between these two groups. Results: forty-two patients (median age 78.5) and 85 patients (median age 70.1) were included in the elderly and the younger groups, respectively, with similar baseline characteristics excepted that the former was most frequently in first line. The toxicity profile was similar in both groups. The median OS was 9.34 months (95% CI, 7.87-10.82) in the elderly group and 6.95 months (95% CI, 5.05-8.85) in the younger group (p=0.28). The ORR (27% vs. 17%, respectively, p=0.24) and median PFS (5.84 months, 95% CI, [4.23-7.44] vs. 5.48 months, 95% CI [4.75-6.20], respectively, p=0.92) were similar in both groups. Conclusion: gemcitabine plus nab-paclitaxel chemotherapy achieved similar efficacy and tolerance in elderly patients and should be considered a standard chemotherapy in this population
