Le dépistage de l’ostéoporose en soins primaires en Gironde : recueil des pratiques professionnelles déclarées et des freins à la prise en charge

Abstract : Introduction: general practitioners are first in line for early screenings. Osteoporosis is a common disease with potentially, severe complications. It is estimated that one woman in three and one man in five over the age of 50 will be affected. The number of osteoporotic fractures keeps increasing despite the fact that osteoporosis has been qualified as a public health issue by The World Health Organization for several years. Considering the aging population, screening is now a public health concern. Material and method: the primary objective of this work was to identify the difficulties and impediments encountered by the primary care physician in their daily practice on this specific matter. The secondary objective, was to highlight the potential need to improve the medical care. Results: sixteen doctors were interviewed. The results of the interviews show that osteoporosis is identified as a serious illness. The impact of complications such as osteoporotic fractures isn’t always appropriately estimated but it wasn't minimized either. Osteoporosis screening is considered as secondary in comparison with diabetes or cardiovascular diseases. Physicians also acknowledge a lack of knowledge and interest in this matter, which, to them, lacks clear diagnostic or therapeutic guidelines. Furthermore, few of them declared themselves fully convinced of the benefits of osteoporosis screening itself. However, they weren't opposed to improved training and assistance in treating this condition. Conclusion: as it appears to be the case on both national and international lever, osteoporosis is largely underscreened in Gironde. Identifying the problems and difficulties can act as a starting point for the improvement of care. Remaining aware of general practitioners' working reality is hugely important in this process as they are the first physicians involved in early screenings of osteoporosis.
