Évaluation des déterminants de consultation aux urgences de Mont de Marsan des patients se présentant de vingt heures à minuit pour des actes réalisables en ambulatoire

Abstract : Goal: over the past 20 years there has been an increase in the number of consultations in the emergency services. At the scale of the city of Mont de Marsan this represents a growth of 69% in 12 years. One of the supposed reasons is the increase of the primary care carried out in the emergency department and in particular during the hours of care. The objective of our study is to highlight the motivating determinants patients to consult in the emergency services rather than to the doctor of call of the permanency of care. Material and method: for eight evenings, from February 18 to May 21, 2018, between 8 pm and midnight, we interviewed the patients presenting to the emergency department of Mont de Marsan and reporting to primary care, on the reasons leading them to consult the emergency department rather than the doctor of permanence and urgency by comparing it to the Emergency Degree Assessment (CIMU classification) by the Emergency Reception Nurse. Results: of the 81 patients included in the study (120 patients who consulted the emergency room during the eight evenings), 56 (69%) consulted emergency rooms without prior medical advice, 15 (19%) were referred by their general practitioner and 10 patients (12%) were addressed by the SAMU regulation. Sixty-five percents of the patients questioned were unaware of the existence of a GP and 40 patients (75%) would have asked if they had known about it. Regarding the 28 patients who were informed about the permanence of care and who did not use it, slightly less than half (46%) thought they had a pathology requiring hospital care, the other arguments were the geographical proximity of the hospital, a council of the regulation, the lack of confidence as well as the absence of answer of the permanence of care. Conclusion: this work highlights the possibilities of improving the care of patients in primary care, including better information on the provision of care in the Mont de Marsan area or better coordination between general practitioners and the use of care and specialized emergency consultations.
