Modèle stress–récompense–mentalisation de l'anorexie mentale : une approche développementale et intégrative de l'anorexie mentale à l'adolescence basée sur les Research Domain Criteria (RDoC)

Abstract : Introduction: the National Institute of Mental Health has developed the Research Domain Criteria (RDoC) to explore the vulnerability factors involved in mental disorders, freeing themselves from categorical classifications. The clinical features associated with anorexia nervosa suggest the existence of common vulnerability factors with other disorders. We propose in this study to explore these vulnerability factors by relying on the RDoC. Method: we conducted a literature review that allowed us to select three areas of RDoC that would be relevant to describe the vulnerability in anorexia nervosa: negative valence (stress), positive valence (reward), and social processes (mentalization). We the PUBMED databases, using the following key words: RDoC, anorexia nervosa, adolescent, mentalization, stress, reward, attachment. Results: our review reveals arguments in an alteration of the processes of stress regulation, reward, and a failure of mentalization in anorexia nervosa. The data allow us to conceptualize a stress - reward - mentalization model explaining the onset of symptoms as adaptive mechanisms to a stressor: adolescence. Conclusion: the alteration of the three domains of stress - reward - mentalization explains the occurrence of symptoms of anorexia nervosa in adolescence. The failure of mentalization processes makes it a preferred therapeutic target that should be explored.
