La prévention du syndrome du bébé secoué par les sages-femmes libérales : étude qualitative auprès de 19 sages-femmes libérales d’Île-de-France

Abstract : Abusive head trauma (AHT) also known as shaken baby syndrome, still remains in the French society but is yet 100% preventable. The midwife, decisive actor in public health because of her closeness to women and infants, seems an obvious choice to ensure this prevention. Objective: We focused on liberal midwives’ point of views and experiences on AHT’s prevention. The main objective is to approach the complexity of prevention by liberal midwives on the theme of AHT but also to assess the barriers. The secondary goal is to suggest ways of improvement in the AHT prevention. This study shows how liberal midwives get involved in the prevention of a such topic, and this, facing various difficulties. Methods: This qualitative study has been held using semi-structured interviews, with 19 liberal midwives among the region of Ile-de-France, on a five months period (between 28/06/16 and 26/11/16). The analysis has been realized using the grounded theory. Results and conclusion: Our study brings to light difficulties faced by liberal midwives in their practice. These seem not to be absolutes barriers to get involved in AHT prevention. On the contrary, midwives show creative ways to overcome them and prevent such a risk. They realize an undirected prevention, anticipating the four factors identified as precursors in gears of AHT, namely baby crying, tiredness, guilt and isolation of adult. Midwives appear to be a key interlocutor to inform, support and guide in the AHT prevention.
