Place de l'arthrocentèse de l'articulation temporo-mandibulaire dans la prise en charge des dysfonctions temporo-mandibulaires : étude d'une cohorte de 5 patients

Abstract : The management of temporomandibular dysfunction (TMD) typically involves four therapeutic areas: physiotherapy, occlusal therapy, pharmacology and psychosocial management. However, a certain proportion of patients do not respond to this treatment. The arthrocentesis of TMJ is a "semi-invasive" technique that involves the puncture of the disco-temporal compartment for lysis and joint lavage. Five cases were reported to be TMD resistant to the “conservative” therapy treated with arthrocentesis. The indications for arthrocentesis are: persistence of localized pain in the TMJ and limitation of the mouth opening. Measurements of mouth opening, pain (visual analogue scale) and quality of life (OHIP-14 test) are performed pre-operatively. Values are monitored at one week and then at six months. One patient was excluded for lack of postoperative consultations. In agreement with literature, our study showed an improvement in all parameters assessed: pain, mouth opening and oral quality of life.
