Etude de la qualité de vie après laryngectomie totale par le biais d’une auto-évaluation

Abstract : The evaluation of the quality of life after a total laryngectomy is essential to pinpoint the patient’s difficulties and thus determine appropriate therapeutic axes. Total laryngectomy is a maiming surgery which inevitably affect the quality of life. This study is a revision of a previous selfevaluation about the quality of life of patients who have undergone total laryngectomy, that have been carried out in 2015. The revised study involves thirty one total laryngectomees and ninety three control individuals. Statistical analysis of the results confirms, or not, the role of independent variables in the alteration of the quality of life. During the first six months post-operation, the quality of life of total laryngectomees is more affected than five years after operation, regarding the orofacial sphere. Moreover, tobacco consumption increases the alteration of the quality of life. On the contrary, neither unfavorable socio-economic factors nor additional treatment seem to significantly affect the quality of life. The benefits of a speech therapy or a postoperative follow-up have not been demonstrated. This might be due to the small number of participants to the study. The same bias could also apply to the acquisition of a rehabilitation voice. It might be interesting to complete the study with a larger sample of individuals, in order to confirm or not these hypotheses, and evaluate the significance of the test. The self-evaluation of one’s quality of life could enable to assess the progress since the beginning of a speech therapy and more easily set objectives in terms of the patient’s quality of life
