Pronostic des réhabilitations prothétiques par implants zygomatiques des sujets partiellement ou totalement édentés : revue systématique de la littérature

Abstract : Prosthetic rehabilitation of the atrophic edentulous maxilla with implants is a therapeutic challenge. Initially developed for patients with extensive defects of the maxilla caused by tumor resections, or congenital defects, zygomatic implants have seen their scope of application extended and seem now considered as an effective alternative to bone grafts. The purpose of this study is to evaluate the outcomes of zygomatic implant-supported prosthesis, which seems to be less reported than outcomes of zygomatic implants, and satisfaction of patients treated with this technique. Materials and methods: A systematic review of studies published from 2014 to 2018 and listed in the PubMed database was performed in accordance with the PRISMA statement, results will be compared with previous systematic reviews. Results: After removing duplicates and preselection of titles and abstracts, 9 articles including 317 patients and 828 zygomatic implants have been selected. The cumulative survival rate (CSR) was 98,5% at 8,4 years. The prosthesis success rate appears to be 100% after 4,3 years. Discussion: Implant and prosthetic outcomes, similar to previous studies, seem to confirm the efficiency and reliability of this technique. However, caution should be taken due to heterogeneity of the reported data, particularly at the prosthetic level or patient satisfaction, and therefore difficult to interpret and compare. Follow-up periods are still too low (<15 years) and management of prosthetic complications, however real, is rarely mentioned. However, zygomatic anchorage appears as a reliable technique for osseointegration and the possibility of immediate loading improves quality of life for patients and makes it a great alternative in oral rehabilitation of patients with severe maxillary resorption.
