Étude des évolutions de la mortalité par cause médicale en France entre 2000 et 2014

Abstract : Objective: this study aims at analysing the distribution of mortality in 2014 and its evolution since 2000 by cause of death and by sex, taking into account evolutions linked to data collection and methods of production. Method: data is collected from death certificates completed by the physician upon death. A generalized additive model is used to detect and assess the value of the level jumps by death category for the year 2011, year of the change of the automatic coding software. A multiple imputation of the unknown causes of death is carried out, then a GEE model with correction of the 2011 level jump makes it possible to estimate trend of the death rates by cause of death and sex between the periods 2000-2007 and 2008-2014. Results: in 2014, 544,740 deaths were recorded in metropolitan France. The standardised all-cause mortality rate decreased by 25% between 2000 and 2014 (from 1150.0 to 862.3 per 100,000 population). The largest relative increase in effectives after imputation is found for transport accidents (+ 16.5% in 2014). The largest decrease between 2000-2007 and 2008-2014 is found for AIDS and HIV diseases for men (RR = 0.54 [0.43 - 0.67] p <10-4) and the largest increase was found for lung cancer for women (RR = 1.32 [1.28 - 1.36] p <10-4). Conclusion: overall mortality is decreasing, but its evolution for some causes of death is to be monitored. Consideration of temporal sources of bias (production changes, increase in unknown causes of death) is necessary for analysis of mortality variations.
