Évaluation des variations des capacités attentionnelles chez les internes en médecine générale au cours d’une plage de travail

Abstract : Introduction: the interns in general medicine are led to practice working ranges of twenty four hours during their various internship. The aim of this study is to evaluate the variations of their attentive abilities at different times in their working ranges. Methods: from June 2017 to June 2018, the interns of Aquitaine were asked to participate in this study. They have completed a booklet containing the different working ranges (day and 24 h). The primary endpoint was the result of the TMT between the beginning and the end of the working range. Incidentally, we assessed the state of alertness and emotion of subjects through the Thayer scale, as well as the stress and fatigue reported by a numerical scale. Results: on the seventeen participants, the half is in emergency and in first year of interteship. Sixteen questionnaires were analyzed, representing forty-eight working ranges. In the overall results, interns put less Test 3 (-3.58 [-6.91; -0.25]). The TMT study shows after a period of work, in Test 4, a higher score (0.94 [0.04, 1.84]). The Thayer's scale has showed a tendency to sleep at the end of the various shift jobs (95% confidence interval never including 0). In addition, a state of stress and fatigue felt by the interns was more important whether it was after a day's work or 24 h (95% confidence interval never including 0). Conclusion: this study suggests that there is a variation in the attentional abilities of a general practice intern over a range of work. However, the TMT is probably not sensitive enough. It needs to be supplemented by more powerful studies.
