Évaluation de la rupture d’information entre le patient, le médecin généraliste et l’équipe hospitalière dans le suivi du glaucome chronique en aquitaine en 2018

Abstract : Introduction: glaucoma is a chronic degenerative neuropathy affecting the optic nerve and responsible for a progressive decrease in visual field that can lead to blindness. It is the second leading cause of blindness in France. Its treatment is mainly by medication and daily instillation of ophthalmic topical treatment (eye drops). The initaition of the treatment and follow-up of the disease is performed by the ophthalmologist, but the renewal of prescription can be carried out by the patient's attending physician. Hence the role of the general practitionner is predominant as coordinator in the follow-up and adherence of the treatment. Method: the purpose of the study was to evaluate the transmission of glaucoma status information from the patient to the attending physician or medical team during hospitalisation. The secondary objective was to assess the spontaneous bringing of his treatment by the patient at hospital and the number of patients who didn't received any treatment during a hospital stay. The evaluation was carried out by a questionnaire given to each glaucoma patient during the visit of the ophthalmologist. Results : 944 patients were included in the study, with a mean age of 68.1 years. , 87% of patients provided the information about the disease to their general practitionner. 164 glaucoma patients with eye drops were hospitalised during the year, 80% of whom informed the medical team and 66% brought their treatment with them. Among the patients who did not inform the medical team and did not provide treatment, none received hypotensive treatment. There was a significant between the information provided by the physician and the information provided by the hospital (p < 0.001). Conclusion: this study shows that one to two out of ten patients do not receive hypotensive treatment during a hospital stay. Patient information and communication between general practionner, ophthalmologist and hospital is crucial to improve glaucoma care.
