Intérêt du bilan biologique standard dans le diagnostic d'orientation de la dengue, du chikungunya, du Zika et de la Leptospirose en zone endémique (Polynésie Française, région Pacifique)

Abstract : Introduction: Differential diagnosis between acute phase dengue, Zika, chikungunya and leptospirosis have been challenged because of a similar clinical presentation and the lack of available molecular diagnosis tools in most endemic areas. Objectifs: Our study aimed to evaluate the use of simple laboratory parameters to differentiate these infections. Methods: We retrospectively compared simple hematology and biochemistry values in patients with confirmed acute phase dengue, chikungunya, Zika and leptospirosis, collected from Avril 2013 to December 2014 during French Polynesia outbreaks. Results: In univariate analysis, elevated C-reactive protein (CRP) was associated to chikungunya and leptospirosis; elevated aspartate aminotransaminases to dengue; elevated alanine aminotransaminases to leptospirosis; elevated creatininemia to chikungunya; leucopenia, neutropenia, lymphopenia and monocytosis to dengue; thrombocytopenia to dengue and leptospirosis. In multivariate analysis, there was a statistically significant association between leucopenia, neutropenia and thrombocytopenia in dengue; no association in Zika; elevated CRP in chikungunya, thrombocytopenia and lymphopenia in leptospirosis. Conclusion: Hematology and biochemistry parameters can be useful to differentiate these infections in acute phase but laboratory confirmation using diagnosis tools remains necessary.
