Enquête de pratique sur la prise en charge d’une plaie post-traumatique en médecine générale en station de montagne

Abstract : Background: Wounds are common. Analysis of literature didn’t find any French recommendations concerning the management of wounds by general practitioner (GP). Aim: determine the rate of wounds managed by GP in mountain stations requiring specialist advice or referral to emergency room. Methods: Observational, descriptive, prospective, quantitative study by questionnaire from December 2018 to March 2019. GP in mountain French stations and wounds secondary by traumatism were included. The primary end point was the decision to send a patient to a colleague who was not a GP. Results: 46 GP (16 %) answered 117 questionnaires. They were men (70 %), aged less than 34 years old (54 %), associated (93 %), moved away from more than 30 minutes an emergency room (76 %). The wounds affected adult (74 %), were not injured from any bite and treated within 12 hours (96 %). They measured less than 10 cm (95 %), had regular edges (82 %), looked clean (95 %) and were deep (46%). The explorations of wounds were evaluated as normal (82 %). 18 wounds (15 %, CI 95 %, [9 %, 22 %]) needed to be examined by another medical doctor, other than a GP. The decision was significatively correlated to the localization, size, depth, cleanliness, and results of explorations of wounds (p < 0,05); and mainly justified by functional risks (83 %). Conclusions: The decision to address a patient to a colleague depended on the wound it-self. Trained, equipped and organized, GP can manage 85 % of wounds.
