Comparaison des performances en fluence verbale de sujets atteints de la maladie d'Alzheimer et de sujets témoins appariés, intérêt d'une épreuve de fluence dirigée

Abstract : Verbal fluency tests are used to assess language and executive functions in Alzheimer's Disease (AD). An early deficit in semantic fluency has been demonstrated in these patients. However, the classic semantic fluency test does not allow us to know how affected each component is. Therefore, we used a directed semantic fluency test, specifically testing the lexical stock.
We recruited 10 subjects with AD (mild/moderate stage), and 10 control subjects (matched by age, gender, and education level), to determine the value of the directed fluency test, to compare their fluency performance and study the effect of the variables.
Our results showed that the mean value of the two groups in the directed fluency test was similar to that of conventional fluency (in directed fluency, control subjects had an average of 32.2, compared to 30.5 in conventional fluency; subjects with AD had a mean value of 14 in directed fluency and of 11,2 in conventional fluency). The absence of any difference between the two tests allowed us to deduce that executive functions and lexical stock were jointly affected on our sample. Subjects with AD had lower fluency performance than controls. Age, gender, and education level had no effect on performance. The MMSE score tends to have an effect. Further research is needed to develop our work and more specifically to assess disorders in the early stage of AD.
