Que font les médecins généralistes dans la prise en charge des patients atteints de cancer selon les oncologues et spécialistes d'organes ?

Abstract : Cancer is the first cause of death in France, its management is multidisciplinary and can involve many doctors, including the general practitioner. This thesis is part of the COCANGEN study which objective is to analyze the activity of the general practitioner in the different fields of action of oncology. Objective : describe what general practitioners do for cancer patients according to oncologists and organ specialists. Method : a qualitative methodology for the collection and analysis of data has been put in place. The choice of the semi-directed interview was chosen by the scientific committee of the COCANGEN study. The data were recorded, transcribed and the main ideas were extracted from each verbatim in open, axial and then selective coding. All of these data benefited from double coding. Results : the actions of the general practitioners described by the specialists were thus: the screening, the diagnosis, the realization of the first investigations, the announcement, the referral in the care sectors, the collaboration with the specialists, the medical monitoring during and after the illness, the accomplishment of administrative tasks, the care of the relatives, the psychological support, the care of the other pathologies, the training, and the management of the end of life. Some of these actions have not been described in the literature so far. A thesis carried out in parallel, questioning GPs about their practice finds results almost superimposable, demonstrating a good knowledge of the field of action of general practitioners by oncologists and organ specialists. Conclusion : GPs have a central role in caring for the cancer patient. The main point of difficulty described in the collaboration between generalist and specialists remains the communication, it would be interesting to develop a research on the obstacles and facilitator of this communication.
