Les médecins généralistes face au dépistage du VIH dans la région Auvergne-Rhône-Alpes : quelles pratiques en 2018 ?

Abstract : Introduction: About 30,000 people are HIV-positive in France without knowing it. The latest HAS 2017 recommendations advocates targeted screening of high-risk populations. The aim of this study was to assess general practitioners' practices and their feelings about HIV testing to verify their adhesion to the recommendations. Method: Between 31 August 2018 and 14 January 2019 a questionnaire was sent by mail to a sample of general practitioners in the region. Those who could be contacted in three different ways, per call, via the social network Twitter, by mailing list of the CPAM of their department. All data was collected via the Limequery platform and statistical analysis was performed via StaView. Results: 334 general practitioners were included. 53% of them prescribed 1 to 4 serologies per month. The majority (75%) felt comfortable with the screening proposal. Half of them (50%) prescribed the serologies both on their own initiative and on that of the patient. The situations for which they prescribed screening were: IDU (95%), change of partner (91%), MSM (86%), pregnancy (81%), origin endemic area (78%), recurrent HSV (75%), precariousness (45%) herpes zoster (44%), pneumonia (18%), seborrheic dermatitis (12%). The majority said they wanted to use the rapid tests if they had opportunity (87%). Conclusion: The French recommendations advocate a strengthening of targeted screening. Although GPs feel mostly concerned about screening, the recommendations of the HAS are only partially followed. The use of rapid tests in general practice seems an option to improve HIV screening.
