Le traitement orthodontique : peut-on réellement l'accélérer ?

Abstract : After describing the cellular mechanism involved in tooth movement, this work will review the treatments that can accelerate the orthodontic treatment. The desire for shorter orthodontic treatment comes from the will for orthodontists to avoid side effects and to match patient demands for the shortest treatment, without compromising its quality. Several technics will be described: micro surgery, injection of chemical stimulants, and physical intervention. Randomized clinical trials and meta-analysis are numerous about micro surgery. This technic is the most used, piezocision in particular, and it was proven that it accelerate tooth movement during the alignment phase. Chemical stimulants are molecules or hormones that have a role in bone metabolism, and are injected locally or IV. D3 vitamin, parathyroid hormone and RANKL are examples of these chemical stimulants. Our knowledge about molecular phenomena involved in tooth movement are getting more precise, leading to more research on the subject. Physical interventions, such as vibrations and pulsed light, are widely commercialized by pharmaceutical companies, claiming its accelerating and pain-killing effect. For those two last technics, the lack of studies and the weakness in their design forbids us to conclude on their accelerating effect.
