Altérations métacognitives dans les troubles schizophréniques : exploration des bases comportementales et neurophysiologiques de la confiance dans une décision perceptuelle

Abstract : Metacognition, or « cognition about cognition », is a reflexive process that allows for the evaluation of mental operations for accuracy. Deficits in synthetic metacognition have been described in schizophrenia using mostly narrative assessment and linked to several key symptoms. Nonetheless, impairments in monitoring and controlling performance in schizophrenia patients is yet to be empirically established. Metacognitive accuracy in patients with schizophrenia or schizoaffective disorder (N=11) was assessed on a visual decision-making task and their performance compared to matched healthy control participants (N = 20). A novel signal detection theory approach was used to model metacognitive sensitivity independently from objective perceptual performance. Behavioral (reaction time, motor response trajectory tracking) and EEG recordings were acquired. We did not find any significative difference between patients and healthy controls as for metacognitive sensitivity. Reaction time for the visual task was more tightly linked to confidence in healthy controls than patients. We found a correlation between confidence and prefrontal EEG amplitude before initiating response motion in controls but not patients. Proof for the inability to accurately attune confidence to perceptuel decision making in patients with schizophrenia remains to be experimentally demonstrated, along with the way such impairments may underpin functional deficits. Exploring behavioral and neural correlates of confidence points to different processing of decisional indices in patients and prompts to further research efforts.
