Variabilité de la fréquence cardiaque et symptomes négatifs dans la schizophrénie

Abstract : Introduction: heart rate variability (HRV), as a reflection of the autonomic nervous system (ANS), could be an index of emotional and social regulation which is an essential component of the negative symptoms in schizophrenia. The medial prefrontal cortex (mPFC) seems also involved in the regulation of HRV and is a treatment target by repetitive transcranial magnetic stimulation (rTMS) of negative symptoms. Our study questions firstly the existence of links between HRV and negative symptoms and also the ability of rTMS to modulate the VFC. Materials and Methods: eleven patients with negative symptoms were included in a randomized double blind study, divided into 3 groups according to the intervention (active rTMS on the mPFC, on the dlPFC, or placebo rTMS). rTMS was delivered by intermittent theta burst twice daily for 5 days in a row. Negative symptoms were clinically evaluated by 5 scales: PANSS, SANS, SNS, V-SIR and MASC before and after treatment. HRV was measured by ECG before and after each rTMS session. Results: a negative correlation was found between SNS (self-assessment) and VFC. None of the 3 groups showed HRV modulation after rTMS. Conclusion: for the first time, we demonstrate the existence of a link between HRV and negative symptoms of schizophrenia only measured by selfassessment. This result could imply a correlation between the ANS and the theory of mind or the subjective dimension of emotions. rTMS did not show the expected effect of HRV. We suggest studies on a larger scale should be conducted to confirm these findings.
