Comment les internes de médecine générale acquièrent les aptitudes nécessaires à l'expression et la prise en compte des préoccupations du patient ?

Abstract : Context: with the health democracy concept, patients ask their doctor to be competent, more particularly in terms of relations. In that sense, the National College of Generalist Teachers has established a new model of general medical SDS (Specialized Diploma of Study) aiming at a competency-based learning for the certification of students. Object: understand how general medical interns learn know-how and social skills related to relationshipcommunication and patient-centered approach. Method: qualitative study by semi-structured interviews conducted with ten general medical interns in Basse Normandie who had hospital and ambulatory experience. Results: the interns had a global biopsychosocial approach with patients and noticed the importance of establishing a quality doctor-patient relationship. They have acquired relational and technical skills through their experience. They all considered that they had made progress during their internships but deplored the lack of communication training, the lack of clarity in the end of internships evaluation grids and the lack of ways given to them to improve. They noticed a difference between hospital and ambulatory patient’s approach and several factors influencing their listening ability. Conclusion: the study revealed that the necessary skills to allow a patient to express himself was acquired by experience. The setup of clear theoretical tools and training in communication appeared to be essential. The evolution of the General Medical SDS with a competency-based certification is a first step towards that change.
