Influence de l'utilisation du jeu en ergothérapie sur la douleur de l'enfant grand brûlé

Abstract : Severe burn is one of the most traumatic accident a child can experience, and it mainly affects children under five years old. These injuries as well as their management generate pain recognized as some of the most distressing ones. And this painful experience can have an impact on rehabilitation and daily life. The aims of this study were to explore the practices of occupational therapists in pain management with children with severe burn, and to analyze the role of the game on the feeling of pain, and thus answer to the following question: What are the contributions of the game on pain reduction in children with burn injuries, during the first occupational therapy rehabilitation sessions? A questionnaire was sent to occupational therapists working in pediatric burn centers. It included about twenty multiple-choice questions, on their practices and their observations, during the first rehabilitation sessions. For all of the observed children, their pain had an impact on their participation in the session. And most of the occupational therapists observed fewer signs of pain and more signs of well-being in children when they were playing. This study highlights that occupational therapists are aware that pain management is an important part of their role, and that using games could reduce the effect of pain. So, why not prescribe occupational therapy sessions specifically to control pain by using games? Thus, the more the child will play, the less he/she will have pain, and the less likely he/she will be to memorize this pain. It would be interesting for a therapist to repeat this study by visiting the pediatric burn centers to observe the children and assess their pain before and after the activity, and on a larger sample.
