Critères de réalisation et de qualité du courrier de sortie d’un patient suite à son passage aux urgences de l’hôpital de Sallanches : obtention d’un consensus par la méthode Delphi®

Abstract : Context: Emergency department discharge letters are tools of communication between cities and hospitals. Although they are recommended, they are not always used, and they sometimes do not reach the recipient on time, or at all. The content of these letters depends on their writer and can therefore turn out to be very heterogeneous. Objective of the study: The main goal of this study was to reach a consensus on the drafting and quality criteria of Sallanches’ emergency department discharge letters. Methodology: From January to June 2019, a prospective, monocentric, qualitative, and analytic study has been carried out on a group of 42 experts working in the surroundings of Sallanches - general practitioners, home care nurses, and emergency physicians - using the Delphi® method. The idea was to propose successive questionnaires with items to evaluate according to their relevance using a Likert numerical scale (from 1 to 9). Items were considered valid when their median was inferior or equal to 8 and less than 25% of answers went from 1 to 3. Those which did not reach that consensus were rephrased or modified, then proposed in the following questionnaire. Results: The experts’ participation rate was 100%. After 3 questionnaires, 30 criteria reached a consensus. A synthetic report was put together in order to facilitate discharge letter writing. Conclusion: This study led to a consensus on 30 drafting and quality criteria of emergency department discharge letters. Their implementation will require to work on standardizing practices among the emergency department as well as setting up computer solutions to enable a fast and simple way to draft the letters.
