Révision de prothèse totale de hanche avant 50 ans : causes et résultats

Abstract : Introduction : Youth has been shown to increase the risk of revision in patients who undergo total hip arthroplasty. Little data are available on the epidemiology of young patients requiring THA revision. The aim of this study is to identify the causes and to report the outcomes. Patients and methods : Data from 123 hips in 120 patients who underwent revision THA under 50 years old from 2005 to 2015 has been assessed in a single-center retrospective study, with minimum 2 years follow-up. Mean age at revision surgery was 41 (12-49). Mean follow-up was 7,7 years (2-14). Reasons were aseptic loosening in 49 cases (40%), infection in 26 cases (21%), dislocation in 19 cases (15%), wear in 10 cases (8%), other in 19 cases (15%). Survival, components used, time to re-revision, reason for re-révision were assesed. HHS, WOMAC, PMA, return to work rate (RTW) and UCLA activity score were recorded. Results : Survival rate at 5 years followup and at mean followup of 7,7 years was 74% and 73% respectively. Among 32 re-revision, causes were 17 loosenings (53%); 6 infections (19%), 4 fractures (9%), 3 dislocations (6%), 2 others. WOMAC, HHS, PMA and UCLA score were respectively 20 (0-68), 80 (13-100), 15 (4-18) and 7 (2-10). RTW rate was 80%, mean delay was 7,5 month. Female gender was associated with higher risk of re-revision, lower HHS, PMA and UCLA score (p<0,05). Conclusion : The functionnal results of revision THA under 50 years old allow to maintain a physical and professional activity but show a moderate survival in the medium term. Rerevision rate is higher than previously reported in general population, especially among women.
