Hémorragies intraventriculaires sévères chez les enfants prématurés : survie et devenir neuro-développemental à 2 ans : étude rétrospective au CHU Grenoble Alpes de 2006 à 2017

Abstract : INTRODUCTION: High grade intraventricular hemorrhage (IVH) is a risk factor for neurodevelopmental disorders in premature infants, including motor disabilities with cerebral palsy and neurosensory deficiences. In some cases, particularly for grade 4 IVH, palliative care and withdrawal of life-sustaining therapies may be discussed. The objective of our study was to assess neonatal survival and neurodevelopmental outcome at 2 years corrected age in premature infants with high grade IVH (grade 3 and 4). METHOD: This was a monocentric retrospective study in Neonatal Intensive Care Unit of the University Hospital of Grenoble, France. This study focused on premature patients born before 37 weeks' GA between 2006 and 2017, who were diagnosed with grade 3 or 4 IVH. The primary outcome was neonatal survival and survival at 2 years corrected age without cerebral palsy. RESULTS: A total of 62 patients were included: 28 patients with grade 3 IVH and 34 patients with grade 4 IVH. 48% (30/62) of patients with high grade IVH survived to discharge: survival rates were 67% (19/28) for grade 3 IVH and 32% (11/34) for grade 4 IVH. Among the 24 survival patients whose outcome at 2 years corrected age was known, 3 patients (12%) had a cerebral palsy: 1 patient either 6% (1/16) among the survival patients with grade 3 IVH, and 2 patients i.e. 25% (2/8) among the survival patients with grade 4 IVH. CONCLUSION: Neonatal survival rates for patients with high grade IVH were similar to those described in the literature. Among the survival patients, the rate of cerebral palsy was lower than expected.
