Critères associés à l’échec des implants mandibulaires extra-oraux en titane poreux conçus sur mesure : expérience de 5 ans au CHU de Caen

Abstract : Bone substitutes become usual in maxillofacial surgery. Mandibular porous titanium implants can be used with free-flap reconstruction to improve functional and aesthetic results. They can also be used alone to treat facial asymmetry. However, complications and deposits may accompany their uses. Our study aimed at identifying the causes of failure of these implants in order to secure their uses. Our study included 17 patients who received 19 porous titanium implants over a 5-year period at Caen University Hospital. These implants were custom-made and printed after virtual planning. The causes and factors associated with them were analyzed. There were 10 depositions, of which 8 were attributable to the implant. The causes of these depositions were exposures (4), infections (2) or planning errors (2). Implant’s symphyseal localization and the association with major bone surgery were the two factors that demonstrated a significant association with the failure of the implant. Exposures are often due to fibrotic tissues that are unsuitable for implantation and can be improved by lipofilling. Considering soft tissue during virtual planning and eviction of infectious entry doors is crucial. The failures of these implants are therefore partially avoidable.
