Statut nutritionnel : impact sur la récupération fonctionnelle après un accident vasculaire cérébral chez la personne âgée

Abstract : The stroke is the first cause of handicap, the second cause of dementia et the third cause of mortality in France. Its probability of occurence increases with age. The deglutition troubles secondary to the stroke can quickly generate undernourishment. Undernourishment consequences on the elderly are well known (infection, loss of autonomy, etc.). Our study’s goal was to identify the factors which influence functional recovery (according to the Barthel score) after a stroke on elderly people hospitalized in the post stroke readaptation department and particularly the impact of the nutritional status.
This was a descriptive, retrospective, observational, monocentric study group, that was held from January 1st, 2016 to December 31st, 2017 in the post stroke readaptation care department in the centre hospitalier universitaire of Nice in Cimiez. Data from 145 patients have been analysed. We have classified them in 2 groups depending on their functional recovery (increase vs stagnation or decrease).
In average patients were 81,1 ± 6,6 years old. The group that showed a Barthel score increase was constituted of 70% of patients whereas 30% of patients have stagnated or decreased. Concerning the nutritional status, 79% of patients were undernourished when they entered the department (46% moderate undernourishment, 33% severe undernourishment) whereas 81% of patients were undernourished when they quit the department (61% moderate undernourishment, 20% severe undernourishment). Therefore, 80% of patients have shown an increase or stability of their nutritional status.
Among the factors with influence on the functional recovery, the increase/stability of the nutritional status had a significant impact on recovery (p = .014) and the degradation of nutritional status was a factor of a difficult recovery
