Dépistage des personnes précaires âgées de 18 à 65 ans consultant aux urgences du CHU de Grenoble grâce au score EPICES

Abstract : Between 12 and 15% of the French population are low income population. And it represents up to 45% of people admitted to the Emergency room. It seems to be important to screen these persons in order to improve cares. So the aim of the study is to screen low income population aged 18 to 65 years old admitted in emergency room in Grenoble universitary hospital. It’s a transversal and descriptive study ; all people admitted from 18 to 65 years old were included except if they presented with an exclusion criteria (absolute emergency, illiterate or non French speaking people, confused people, imprisoned or under guardianship people) or if they refused to participate. It took place during five days of 24 hours, drawn. An evaluation form was filled in next to the patient by medical students in the ER or by an investigator. The main objective was to screen the number of low income people aged from 18 to 65 years old admitted to the Emergency Room of the Universitary Hospital, thanks to EPICES score. 46,2% of people admitted to the Emergency Room are low income people. Most of these patients are men (56,7%), young, smokers (59,4%), unemployed (55,6%), of French nationality, lacking dental care for the past two years, with multiple admissions to the ER during one year, for psychiatric or addiction problems more frequently compared to non precarious people. However, we can notice that 45% of low income population have a job, 84% have declared a general practitioner and 82% have health insurance. People affected by precariousness are more frequently affected by chronic disease and association between unemployment and smoking is linked with a risk of 80% to be precarious. There is a large population in France affected by precariousness, they are more or less easy to be detected, due to modified criteria over time. It’s necessary to apply early detection strategies for more adapted and personalized care.
