Prise en charge du surpoids de l’enfant en réseau de second recours : quels sont les facteurs influençant les délais entre le début du surpoids et l’inclusion dans un réseau spécialisé ? Analyse au sein du RéPPOP 38

Abstract : Background: 18% of french children are overweight. Secondary organised care has proved its efficiency, but the patient care delay, between the begining of the overweight status and the start of medical care, is too long and worsen the prognosis of overweight children. The goal of this study is to analyse the differences between the children who benefit from an early intervention compared to a delayed intervention by the RéPPOP38. Method: A controlled case study was conducted on patients included between april 2010 and june 2018. They were under 18 years old, overweight, had no previous experience with RéPPOP and lived in Isère. A logistic regression was realised on 1457 files. Patients were divided into 2 groups based on the median period of time before they get care (64 months) ; 746 were included before that time and 711 after. Results: Before 64 months of evolution they had a lower BMI Z score (OR 0,56 IC95 [0,46-0,70]) and were more often sent by PMI (OR 0,24 IC95 [0,09-0,53]). They suffered from less complications related to excess weight (OR 0.31 IC95 [0.20-0.48]) and became overweight older (OR 0,95 IC95 [0,94-0,96]). After 64 month of evolution, the first consultation was triggered by sport practice issues (OR 1,66 IC95 [1,23-2,25]) or a discomfort based on physical appearance (OR 3,23 IC95 [2.37-4.43]). They were more likely to repeat school (OR 2.34 IC95 [1.49-3.71]). Studied socio economical factors and family weight status were not statistically different in these 2 groups. Conclusion: Studying the datas of other french RéPPOP and qualitative studies would enhance conclusions and allow us to improve care.
