Validation de méthode du CASA (Computer-Aided Sperm Analysis) Hamilton Thorne CEROS II pour son application en routine dans un laboratoire de spermiologie

Abstract : Semen analysis is fundamental in the assessment of male infertility. Despite high inter- and intra-operator variability and the lack of standardization, the standard manual sperm analysis is still the gold standard (WHO 2010). In the context of mandatory accreditation for any biomedical laboratory, emergence of computer aided semen analysis (CASA) systems is considered as a more precise alternative. The aim of this study was to evaluate the CASA Hamilton Thorne CEROS II® system performance compared to the standard manual semen analysis according to the WHO guidelines for sperm assessment and the Cofrac accreditation guidelines. Repeatability, intermediate precision and accuracy were all acceptable. Uncertainty of measurement was about 1,5 to 6,0 millions/mL for sperm concentration, 2,2 to 6,1% for total motility and 2,2 to 8,1% for progressive motility depending on the values. Furthermore, Bland-Altman plots and regression analysis revealed good correlation and agreement between CASA system and standard manual semen analysis. Potential interferences were identified and their impact can be controlled using risks management. In conclusion, CASA system achieves satisfactory performances, better than manual analysis, notably through the flagellum detection in the algorithms. In addition, CASA provides required qualities for Cofrac accreditation easier than a subjective manual method with high inter-operator variability. Moreover, CASA enables standardization, data traceability and inter-laboratory comparison.
