Les motifs de recours des courriers des médecins généralistes et leur réalisation aux urgences

Abstract : Introduction: 20% of the 21 million annual emergency department visits are accompanied by a referral letter. This letter could be better used as soon as the patient arrives in the emergency department. The objective of this study is to analyze the reasons for sending patients to emergency departements exposed by general practioners in referral letters and how their request is carried out. methods: Data collection of all adult patients referred by general practitioners to the Annecy emergency services with a letter, during the winter of 2018-2019. Analysis of the reasons of referral to the emergency department (hospitalization request, diagnostic hypothesis, request for resources, specific therapeutics, symptom chart, social needs) and how the requests are fulfilled. results: 1013 letters were collected (12.3% of admissions), 729 requests have been made on the 976 referral reasons ie 74.7%. 132 hospitalizations out of the 164 requested, ie 80.5% vs 385 out of the 826 with another reason, ie 46.6%, (p<0.01). 218 diagnostic hypotheses were confirmed out of a total of 333, ie 66.3% (p <0.01). 54 specific treatments were performed on the 62 requests, ie 87.1% (p<0.01). Biological analysis were prescribed for 163 cases (94.8%), medical imaging for 193 patients (76.9%) and specialist advice (160 cases), ie 74.8% (p<0.01). conclusion: There is a good match between the reason of referral exposed in the letters from general practitioners and the fulfilment of this request in the emergency department, even more if it concerns an hospitalization. The referral letter influences the patient's pathway to the emergency room and could be used as an early medical prescription on arrival.
