Dynamique faciale : cartographie des contractions faciales normales et synergiques physiologiques

Abstract : Introduction: Facial palsy sequelae such as synkinesis, hyperactivity and spasms, are responsible for pain and quality of life alterations. Their treatment by injection of botulinum toxin requires a better understanding of facial dynamics. Several pathophysiological hypotheses explain these synkinesis. We hypothesize that a part of it is due to the exacerbation of physiological synergistic contractions.Objectives: The main objective is to create a map of normal and synergistic contractions and to compare them with synkinesis. The secondary objective is to study the overall contractility of faces.Materials and Methods: Our monocentric study involves 52 healthy subjects from the University Hospital of Grenoble. Each facial muscle is evaluated using photographs and videographs in ten defined movements. Results: A mapping of normal and synergistic contractions is performed. We do observe similarities with the synkinesis described in facial palsy, suggesting the validity of our hypothesis; as well as co-contractions not yet described. In addition, our study highlights functional muscle groups, and statistical analysis allows us to define three phenotypes according to the overall intensity of contractility. This increases significantly with age and gender. Conclusion: The analysis of synergistic contractions seems consistent with the prevalence of synkinesis in facial palsy. They are, as well as the overall contractility, to be taken into account for the personalized treatment of synkinesis by chemodenervation.
