L’intérêt de la rédaction des directives anticipées et de leur utilisation. Étude auprès de médecins spécialisés en soins palliatifs à La Réunion et en Métropole.

Abstract : BACKGROUND: The Advance Directives (AD) were amended in 2016 by the Leonetti-Clayes Law on End-of-Life Rights. They have become opposable to doctors and have an unlimited duration. Their writing rate remains low. Moreover, the advance directives currently in place do not seem to be of much help to physicians in their decision-making. This work asked palliative care specialists about their opinion and perception of ADs. Its objective is to understand the place of DA writing in their practice but also to gather their opinions on their use in France. METHOD: The study was conducted through semi-structured interviews with palliative care specialists practicing in palliative care. In order to gather their opinions on the place of the drafting of the advance directives in their practice as well as the use that is currently made of them. The interviews were conducted in person or by telephone, recorded, anonymized, transcribed verbatim and then subjected to a general inductive analysis. RESULTS: 17 doctors were interviewed working in Reunion Island and metropolitan France, most of whom reported a strong interest in the discussions on AD between the patient and the doctor. However, this system occupies a small place in their practices. CONCLUSION: AD seem to be a good tool for discussing end-of-life issues but do not seem necessary for all patients. They require a relationship of trust, an end-of-life experience and time to be supported. Increasing the frequency of their writing may not be the only objective to make them more useful.
