Préparation du retour à domicile pour des patients en EVC ou en EPR : une dynamique de collaboration entre la famille et l’ergothérapeute

Abstract : Background : Persistent vegetative state (PVS) or minimally conscious state (MCS) patients have a severe level of dependency. In this situation the family is in great pain, because their relative is still alive but with a consciousness disorder. In this complex pathway of care, some families wish to take care of their relative at home. For this purpose, the care team, including the OT, has to support and assist the family’s choice, but always in the well-being of the patient. This study’s aim was to understand how the collaboration between the OT and the family member promotes the homecare of patients with PVS or MCS? Methods : A qualitative method was used for this research. The interviews’ questions were built on theoretical inputs which are the PPH-2 model, the collaboration and the systems theory. Results : Three OTs were interviewed. All had already or actually prepared a home return. They agreed on the importance of letting the family make the request. One of them talked about a relationship based on collaboration and preparation. In all three cases, the financial aspect was essential, one of the OTs insisted on the fact that it could lead to the end of the project. Finally, they all admitted that families could be as helpful as obstacle to the preparation of homecare. Conclusion : This research highlights different determinants of collaboration. The major bias was the absence of family caregivers’ points of view. It would be interesting for further studies to interview them
