Étude de la morbidité materno-néonatale selon la voie d’accouchement chez les femmes porteuses d’utérus bicicatriciels. Une étude de cohorte rétrospective au CHU de Robert Debré

Abstract : Objective: our objective was to compare maternal and/or neonatal morbidity according to the delivery mode in a population of patients with history of 2 cesareans. Method: we performed a retrospective cohort study in a type 3 universitary hospital. We included women with history of two cesareans and who gave birth between January, 1st 2001 and December, 31st 2018 of a single live term eutrophic fetus in cephalic presentation. The mode of delivery was studied as follows: vaginal brith (VB), Cesarean during labor (CSDL) and Cesarean before labor (CSBL). The main outcome measure was maternal+neonatal morbidity. The secondary outcomes measures were maternal or neonatal morbidity separately. Univariate analysis using Fisher exact test were performed. Results: of the 716 women with history of two cesareans studied, 627(88%) had CSBL, 65 (9%) had CSDL and 24 (3%) had VB. Our three groups didn’t differ significantly excepted for history of vaginal delivery (p=0.014) and gestational age (p=0.005). Maternal+neonatal morbidity was non-significantly decreased for VB group (12.5%), compared to CSDL (18.5%) and CSBL (18.3%). No significant difference was found for maternal morbidity according to the mode of delivery (p=0.195). VB was associate to a non-significant decrease in neonatal morbidity compared to CSDL and CSBL (p=0.089). Conclusion: our study didn’t show any significant difference in maternal and/or neonatal morbidity according to the mode of delivery among women with history of two cesareans.
