État des lieux des pratiques de soins palliatifs au sein de l’hôpital gériatrique de Cimiez : questionnaire aux équipes médico-soignantes

Abstract : Palliative care are active care delivered by a multidisciplinary team following a global approach of the person suffering from a severe, progressive or terminal disease. Palliative care and end-of-life assistance represent a large part of the gerontological day-to-day activity inside the Rehabilitation Autonomy Ageing department of the University Hospital Centre of Nice, despite not being institutionally recognized for this specific patient care. Medical care teams regularly have to question the purpose of their activity and reflect on the ways to improve the older patients care conditions.
The main purpose of our study was to determine the key drivers limiting the level of satisfaction related to the care of the medical teams throughout the palliative patient care and end-of-life support. The objective being to identify measures to implement in order to improve these factors. We conducted a prospective survey by self and anonymous questionnaire between the 18th of March and the 24th of May 2019. It was directed to the medical care teams of the Geriatric Hospital of the University Hospital Centre of Nice.
Results of the quantitative analysis show that discontent of the caregivers is first related to a too heavy workload in care units. The feeling of a lack of means to relieve patients and a lack of training are, respectively, the second and third sources of discontent. Results of the qualitative analysis show that the main proposals of the caregiver’s team to deal with this issue are: an increase in the material and human resources, the creation of beds allocated to palliative care and a better in-service training.
Our study has shed light on the main sources of discontent among the medical teams regarding palliative care, among which: a too heavy workload, insufficient means to relieve patients, a lack of training and a perfectible organization.
