État des lieux sur les hospitalisations secondaires en médecine néonatale des prématurés tardifs

Abstract : Objectives: The main objective was to determine if obstetrical, maternal or neonatal variables were related to the secondary hospitalisation of late preterm infants (35 - 36+6 gestational weeks) in the neonatal unit of the hospital Nord of Marseille. A second aim was to identify the reason for the hospitalisation. Materials and methods: A single institution retrospective exposed-non exposed study conducted in hospital Nord of Marseille (type III) in France. Studied cases were from 2016 to June 2018. 147 cases were included as they fulfilled the criterion: live and viable birth at 35 - 36+6 SG at this hospital. 52 cases reported a secondary hospitalisation in the neonatal unit versus 95 non rehospitalised. Maternal, obstetrical and neonatal data were compared, as well as the times and reasons for rehospitalisation. Results: Rehospitalisation was significantly linked to maternal drug abuse (p=0,007), controlled diabetes (p=0,041), corticosteroid therapy (p=0,035) during pregnancy and the term of birth (p=0,004), 5-minute Apgar score (p=0,026), neonatal ventilation in the first 2 hours (p=0,042). The reasons for rehospitalisation were from most to least frequent: more than 10% loss of weight, respiratory and eating disorders, jaundice, metabolic disorders, hypotonia, blood sugar disorders, neonatal infections, and diagnosis of malformations. Conclusion: The main objective was reached and results corroborate data from American and Anglo-Saxon studies. This field still opens to many possible studies that remain.
