Un protocole pour intégrer la prise en charge de l’ankyloglossie des nouveau-nés au soutien à l’allaitement en maternité

Abstract : Background: Lingual frenulum is a small fold of mucous membrane attaching the mouth floor to the tongue underside. 4-11 % of newborn have a tongue-tie whish impairs tongue movement required for efficient sucking. Ankyloglossia is reported as the second cause of nipple pain, which in turn causes early weaning in the absence of appropriate management. Frenotomy is established as the standard treatment when breastfeeding difficulties are reported and ankyloglossia is diagnosed. Conservative measures can also be implemented in some cases. In France, there are no recommendations or established procedures for the management of newborn with symptomatic tongue-tie. Aim: Write a Protocol for ankyloglossia management of newborns during hospital stay. Methods: Procedures and recommendations regarding ankyloglossia management worldwide were analysed using a literature review. Screening and assessment tools were also reviewed from the literature. Finally, a retrospective study including 57 newborn diagnosed with ankyloglossia was used to analyse pre-frenotomy symptoms to be included in the assessment part of the Protocol. Results: The newborn ankyloglossia management Protocol is based on recommendations, evidence from studies as well as collected data. It sets a step by step management plan, establishes the tools to be used for tongue-tie assessment and gives follow up recommendations. Discussion: Implementing the Protocol will allow the assessment of practices and management of newborn with tongue-tie. Breastfeeding support as a core competency of midwives will be improved.
