Étude rétrospective et multicentrique de l’association du Brentuximab Vedotin et de la Bendamustine dans le lymphome de Hodgkin réfractaire ou en rechute et revue de la littérature

Abstract : Introduction: The aim of our study was to assess the efficacy and tolerance of the combination Brentuximab Vedotin plus Bendamustine (BV+V) in patients with Refractory or relapsed (R/R) Hodgkin lymphoma (HL) in “clinical routine practice”. Patients and Methods: This a retrospective and multicentre study. Eighty patients (pts) with refractory or relapse (R/R) HL from 15 LYSA centres treated between 2014 and 2017 were retrospectively reviewed. The primary efficacy endpoint was CR after BV+B combination therapy. Secondary endpoints were PR and OR rates, duration of response (DOR), PFS, overall survival (OS), stem cell collection feasibility, outcome after transplant, safety and tolerability. Results: 76 pts were evaluable for efficacy. The CR rate was 68.8%. After a median follow-up of 15,7 months, the estimated 2-year progression free survival and overall survival were 58% (54-52) and 88.5% (84.5-92.5) respectively. After ASCT, complete response rate was 86.5 % (32/37) compared to 51.4% % (18/35) in the group without hematopoietic stem cell transplantation (w/o HSCT) (p=0.01). Median PFS was not reached, compared to 16 months (14-18) in the group w/o HSCT (p=0.033). Median OS was not reached in both groups. Twenty-three patients (28.8%) experienced grade ≥ 2 toxicity, mainly hematologic (13.8%), infectious (10%), cutaneous (3.8%) and digestive (3.8%) toxicities. Treatment was interrupted for 7 patients due to toxicity, with 2 deaths probably related to toxicity. Conclusion: BV+B is an effective salvage therapy in patients with R/R HL. Considering the very good outcome for patients in CR and after ASCT, its use as a “bridge-to-transplantation” therapy should be considered to achieve durable remission in eligible patients.
