Exposition in utero au Valproate de sodium : stratégie thérapeutique dans l’épilepsie et mesure de minimisation des risques

Abstract : Epilepsy and its treatment usually require high caution, particular vigilance is required with regard to epileptic women of childbearing age. Sodium valproate, standard treatment in epilepsy, is sometimes the only effective drug option. However, in utero exposition to valproate may lead to an increased risk of teratogenic effects responsible for physical abnormalities and neurodevelopmental disorders in children. Despite this risk’s knowledge, the concern in the management of epileptic patients is to find a balance between the disease and the risk induced by the drug. For this, a young woman treated with antiepileptics should be closely followed from the planification of her pregnancy to the delivery. It is essential that the benefits/risks ratio must be assessed at each prescription. In order to minimize the risks associated with in utero valproate exposition, several measures have been implemented. Recommendations for the use of these molecules have been restricted and educational materials for health professionals and patients have been developed. Effective communication could guaranty patients and fetus safety.
