Conception et gestion de la maladie : étude exploratoire par focus groups auprès de médecins généralistes

Abstract : In 2015, a quantitative study of disease’s management by GP showed that there was a clear separation between the management of typically acute diseases (eg, angina) and the management of typically chronic diseases (eg diabetes), but also an intermediate zone of chronic conditions for the scientific community, acutely managed by GP (eg, asthma, obesity, addiction). The aim of this study was to explore the point of view of GP on the management of diseases, including this intermediate zone.We made a qualitative study by focus groups between January 2016 and December2016 and elaborated an interview guide based on hypotheses developed following a study of GPs practices and a review on the determinants of disease management by GP. Discussions were centered on their management of a typical acute disease, typical chronic disease, and intermediate conditions. A variety of GP was included by reasoned sampling. The data saturated after 5 focus groups. Thematic and matrix analysis were conducted with transcript of the contents of the verbatim coding with the NVIVO software.The participants identified determinants of disease conception: disease characterization, recurrence or acute event, physician representation, and societal factors. Determinants of disease management by GP were: patient management, consultation management, treatment management, risk management, follow-up management, guidelines, and use of health care system.Specific management determinants characterize acute or chronic diseases, but we also identified an intermediate continuum in which certain pathologies are located, thus calling into question the acute / chronic dichotomy still used in disease management.
