Épidémiologie du profil lipidique dans une cohorte prospective d'AVC à la phase aiguë et implications pronostiques

Abstract : Stroke is a leading cause of acquired disability in France. Serum lipid levels have been investigate as predictive and prognostic factor in ischaemic strokes (IS) with conflicting results. Reunion island population has been reported with high risk of stroke because of high prevalence of cardiovascular risk factors. The aim of this study was to investigate serum lipid levels and apolipoproteins epidemiology in acute stroke in Reunion Island and to determine wether they are associated with functional outcome at 6 months. All the patients admitted from November 2017 to April 2018 in emergency department for stroke suspicion were enrolled. The subjects were classified in 4 categories: IS, hemorrhagic stroke (HS), transient ischaemic attack (TIA) and mimics except TIA. Serum lipid levels were assessed during the first 24 hours and apolipoproteins were assayed at admission by mass spectrometry. One hundred and twenty five patients were enrolled, among them 78 had a serum lipid profile and 84 had apolipoproteins assay. High-density lipoprotein cholesterol (HDL-C) was significantly higher in IS (1.24 mmol/l [0.88 – 1.59]) compared to HS (1.06 mmol/l [1.02 – 1.11] p < 0.001) and TIA (0.89 mmol/l [0.82 – 1.07] p < 0.001) and significantly lower compared to mimics except TIA (1.52 mmol/l [1.04 – 1.70] p < 0.001). The apolipoprotein B to apolipoprotein A1 ratio (apoB/apoA1) was higher in IS compared with mimics except TIA but not relevant. In IS, serum lipid profile and apolipoproteins were not associated with 6 months outcome. Our study show the lack of relevance of serum lipid profile as diagnostic or prognostic factor and suggest apolipoproteins to be a modern and interesting approach for this purpose.
