Revue de littérature sur les échelles validéesde iatrogénie médicamenteuse chez le sujet âgé

Abstract : Introduction : Medication-induced iatrogeny is a public health issue in France, especially for the elderly patients who are often polymedicated. The prevalence of these events is high and can cause severe adverse effects. Hospitalization rates rise, as does the cost of healthcare. For these reasons, using tools to detect potentially inappropriate prescription (PIP), especially anticholinergic burden, could be interesting. . The objective of this study was to identify all PIP scales validated in the elderly population. Method : We conducted a narrative literature review in April 2018, in French and English on Medline, Embase, Cochrane Library and grey literature. We included studies in French or English,describing the elaboration of their PIP scale, which had to be usable in the geriatric population. Result : We found 31 validated scales of PPI in the elderly population, from North America, Europe, Asia, and Australia. Each scale included a list of PPIs. Some described a list of medication interactions or medication/disease interactions to avoid. Benzodiazepines, NSAIs, tricyclic antidepressants and anticholinergics were the most mentioned therapeutic classes in those scales. Anticholinergic burden was calculated in 5 out of 31 studies. Beers criteria and STOPP/START are the most used scales. Conclusion : Many validated tools to detect PPI exist in the elderly population. Most of the mentioned medication / most medicine is commonly prescribed are commonly prescribed in the everyday life. Use of these tools could help reduce medication-induced iatrogeny which could decrease the onset of severe adverse effects, and with that, lower hospitalizations and even mortality.
