Le diagnostic syndromique des méningites et méningoencéphalites infectieuses : exemple du panel BioFire® FilmArray® Meningitis/Encephalitis

Abstract : Infectious meningitis and meningoencephalitis (ME) are serious diseases. They require emergency management, and the administration of an anti-infective treatment without delay. This treatment is usually probabilistic, because the kinetics of evolution of these pathologies do not allow to wait for the results of the etiological diagnosis from the cerebrospinal fluid (CSF). Indeed, with the usual microbiological diagnostic methods, it takes several hours to days to identify the pathogen, moreover, they also have a sensitivity sometimes limited. Syndromic testing consists of the joint and simultaneous research of the principal microorganisms suspected before a given clinical syndrome, by molecular biology (with exceptions). The BioFire® FilmArray® Meningitis / Encephalitis panel is indicated for the etiological diagnosis of infectious meningitis and ME. It allows the search in 1 hour of 14 pathogens (6 bacteria, 7 viruses, 1 yeast), from the CSF, by multiplex and nested PCR. This method is sensitive and specific, and several studies on the use of this panel in clinical practice have shown very promising results on the turnaround time, the reduction of the duration of hospitalization, the therapeutic management and medico-economic evaluation. However, the existence of certain limits and the novelty of this product require additional studies to define its clear position, and more generally that of the syndromic testing, in the diagnostic strategy of infectious meningitis and ME.
