Difficultés rencontrées par les médecins généralistes dans la prise en charge d’une morsure de tique avec suspicion de borréliose de lyme

Abstract : Aim:
The incidence of Lyme borreliosis is increasing in France. General practitioners (GP) have to deal with diagnostic and therapeutic difficulties due to the polymorphism of this disease, but also with the mediatization which generates a growing concern of their patients on the subject.
Design:
Highlight difficulties encountered by general practitioners in the diagnosis and management of a tick bite with suspicion of Lyme disease.
Method:
A qualitative survey was conducted using individual interviews with 11 general practitioners in the department of Haute Savoie (France), according to the "close by" method from May to July 2019. The interviews were recorded, fully transcribed and analysed by axial coding.
Results :
The diagnosis and the therapeutic management of Lyme disease is limited by the imprecise knowledge of general practitioners about the pathology and its vector. Few of them were trained on the subject. The 2018’s recommendations of the HAS on tick-borne diseases do not seem to be a sufficient aid in general practice exercise. The interviewed GPs were not able to respond appropriately to their patients' queries. Their frustration was increased due to difficulty to source information from the specialists resulting in a challenge to provide care.
Conclusions:
General practitioners feel alone when caring for Lyme disease, which is a new public health problem. Patients’ prevention and education seems to be necessary. Specialised centers will improve the multidisciplinary medical care. But specific tools are also needed for consultations in order to help the GPs in their task.
