La schizophrénie et les antipsychotiques injectables à action prolongée : optimisation et coordination de la prise en charge de l'hôpital à l'officine

Abstract : Schizophrenia is a complex chronic disease that requires multidisciplinary care. It represents a public health issue because of its prevalence and its consequences in terms of morbidity and mortality for patients, but also for their families and society. The care pathway of the patient with schizophrenia integrates both pharmacological treatment and non-drug measures: psychological management, therapeutic education, accompanying measures and social support. The current drug strategy of this disease has evolved considerably and is now optimized thanks to the marketing of late injectable treatments which, administered every 15 days, every month or every 3 months according to the active ingredients, facilitate compliance and reduce the risk of relapse. If this global therapeutic approach allows for a quick return to the outpatient clinic and an improvement in the quality of life of the patients, they must benefit from a regular bio-psycho social follow-up, adapted individually to each patient. The management requires all the health professionals involved in the patient care pathway and the pharmacist has a crucial role to play in the smooth running of care as the first referral professional for the patient because of its proximity and its accessibility. Its missions are educational as well as safety. They are conducted as part of the continuity of patient care and hospital-city coordination.
