Diffusion d'un parcours coordonné pour l'arrêt du tabac en ex-Aquitaine : freins et leviers pour l'appropriation de la méthode et des outils par un collectif d'addictologues

Abstract : Introduction: an experiment has been realized in 2017 at the Bouscat Hospital, which aimed to familiarize general practitioners with tobacco cessation accompaniment. The idea was to integrate them, in a practical way, in the care of their smoking patients, shared with the addictologist. The evaluation of the perception and the attitude of the 30 general practitioners implicated showed that these were well received and the improvement of their skills. It has been proposed to diffuse the method and the tools of this initial experience to others volunteers addictologists in ex-Aquitaine. Objective: to identify the difficulties and action’s levers in obtaining an implicated specialist consensus, for the diffusion of this coordinated care experience. Method: it’s about two qualitative studies. The first one on the exchanges in meetings between specialists, during the consensus-seeking, based on the full transcript of the partipants’ discussions. The second one is about the opinion’s collecting by semi-structured interviews. Results: the vast majority of addictologist have joined the issues of this process. However, in practice, the dissemination of the coordinated course was more complex. Thus if the levers were identified (favourable recepetion from health workers, the progressiveness of the approach and the participatory method) many obstacles were also perceived (diversity of organization and practices of the addictologist involved, profile of their active file, complexity of the debates, geographical distance, …) Conclusion: this experiment has been supported by a large number of health workers. The appropriation of the method and the tools used tooks several months. The next step will be to support its implementation on the ground.
