L’innovation médico-sociale dans le domaine de la santé des populations précaires : apport du dispositif Lits Haltes Soins Santé (LHSS) en Nouvelle Aquitaine

Abstract : Social inequalities in healthcare are important in France as morbidity and mortality are high among the homeless population. Given this situation, the « Care and Health Halt Beds » (LHSS) have been created in 2005 to bring medical and social care to homeless or vulnerable patients whom don’t require hospitalization but need housing to achieve care. The objective of this study is to describe innovative practices of the LHSS specialized workers in Nouvelle Aquitaine, after ten years of experience, to improve healthcare of patients in high insecurity. This is a descriptive cross-sectional and retrospective analysis of their practices, extracted from the activity reports of 9 out of 11 regional centres. To understand how their innovative work improves people’s healthcare in precarious state, this study is supplemented by a qualitative analysis of semi-directives interviews from 13 professionals of these structures. N vivo 12 software was used to identify major thèmes proposed by the interviewees. The main results showed a unanimous positive assessment for everybody and a proper functioning but also a need of improvements like increasing the current capacity and delivering medical beds for patients with chronic diseases. The study shows that the LHSS are a secure place that enhance emancipation of precarious people with medical and social history by guiding them to the common law. It is also a place where knowledge and innovation take place which helps stabilising the medical and social community to take care of difficult patients. Their role is essential for the health of the homeless and their management using a more coordinated, efficient and promising care.
