Difficultés rencontrées par les médecins généralistes du Pays Basque Français à propos de la vaccination contre les papillomavirus humains : étude qualitative

Abstract : Introduction: human papillomavirus (HPV) is responsible for many cancers, including cervical cancer, which is a public health problem in France. Since 2006, anti-HPV vaccines are available, however coverage is inadequate. Objective of the study: to explore the difficulties of general practitioners about HPV vaccination. Method: qualitative study in semi-structured interviews. Results: 17 general practitioners were interviewed .The majority of GPs were in favor of anti-HPV vaccination. For some this vaccine did not matter the others. For others there was a lack of hindsight and information. A minority of GPs expressed doubts about potential immune responses. The other brakes specific to GPs were: lack of time, a time-consuming subject without dedicated consultation, forgetting to mention it. The girl's approach to sexuality was embarrassing for the doctor and for the patients. Few GPs knew of the indication to vaccinate MSM. On the patient side, the main obstacles were: the fear of side effects, especially autoimmune diseases, in analogy with the controversies surrounding the hepatitis B vaccine and suspected cases of sclerosis in plaques fueled by the media; mistrust; a lack of hindsight and information, the negative influence of paramedics. GPs then quoted reassuring studies, spoke of Australia where the decline is more important. They argued more than they listened. According to them, it is necessary to sensitize the general public. And vaccination for all boys is a big hit. Conclusion: the training of GPs and nurses on the subject is to be strengthened. The mode of communication is to be reviewed, with a focus on motivational interviewing, which seems more effective than argumentation.
