La prescription d’aspirine en prévention primaire des événements cardiovasculaires : la fin d’un casse-tête

Abstract : Acetylsalicylic acid (AAS) is a drug with recognized indications for secondary prevention of cardiovascular events. Its indication in primary prevention is less clear, because of recommendations and contradictory scientific data, constituting a real paradox and may be at the origin of prescription difficulties for general practitioners in clinical practice. Purpose: to highlight the difficulties of prescribing AAS for primary prevention of cardiovascular events for general practitioners in Réunion facing this "brain teaser"; to know the prescribing habits and criteria of AAS in this indication; understand the role of AAS in primary prevention according to recent scientific data. Method: this was an opinion survey among 500 general practitioners in Reunion by means of an anonymous computerized questionnaire sent by email between January and May 2017. These were descriptive statistics. Results: 50 answer were collected. The results highlighted the existence of a real difficulty in prescribing AAS in primary prevention of cardiovascular events, at different levels. The important heterogeneity of practices about habits and prescription criteria underlines the notion of a brain teaser in clinical practice; all reflecting a lack of clarity of the current recommendations and conclusions of learned societies, often contradictory, being evaluated in a third part. Conclusion: there are real difficulties in prescribing AAS for primary prevention of cardiovascular events for general practitioners in Reunion. The current data of science do not make it possible to maintain its prescription in this context because of non-favorable benefit-risk ratio.
