Influence de la pression inspiratoire maximale sur la capacité d’exercice dans la BPCO sévère à très sévère : analyse post-hoc

Abstract : In Chronic obstructive pulmonary disease (COPD), exercise capacity impairment is usual. Peripheral muscle dysfunction, particularly of the lower limbs is known to affect physical capabilities and exercise ability. Diaphragmatic involvement, closely related to dyspnea, also plays a role in the impairment of physical capacity. Maximal inspiratory pressure (MIP) is reliable and easy to perform in current clinical practice. The aim of this study is to evaluate the link between inspiratory muscle strength and exercise capacity in COPD. Methods: We conducted a post-hoc analysis of the data from the study conducted by Marc Beaumont et al which included 149 severe to very severe COPD patients. Inspiratory muscle strength evaluation was carried out in a standardized way using the MicroRPM® portable pressure gauge. The primary endpoint selected was the distance traveled during the six-minute walk distance test (6mwd). Results: A statistically significant correlation between Pimax (MIP) and the distance walked on the six-minute walk test was found. (r = 0.35, p <0.05) Conclusion: The results suggest the efficacy of this test in the evaluation of exercise capacity in COPD. However, other clinical markers must be used complementarily for the evaluation of this parameter in this pathology.
